Employees may be eligible to use Family and Medical Leave in certain circumstances. Learn more by reading Policy 3430 – Family and Medical Leave.
Request
Write a letter to your supervisor indicating the need for leave, amount of time requested and whether you plan to use sick, vacation, personal days or take unpaid time off. Supervisors should let employees know they are eligible for family and medical leave when needed.
Paperwork
Complete the FMLA paperwork and return it to Nicole.
Return to Work
Once the leave is complete, the staff member returns any needed return to work paperwork and begins work again.
Forward Request
Supervisors should forward the request to Nicole (nicole.barlass@cesa2.org) so she can provide paperwork to the staff member.
Leave
Staff member takes the requested leave.
Update
The staff member records the leave information in Alio as needed.
