Conference Attendance Procedure & Form

Employees interested in attending a conference or workshop should complete the Conference or Workshop Attendance Form below. A notification will be sent indicating whether or not attendance has been approved.

Please note that you must book the hotel yourself for a conference or workshop. Please use the Hotel Reservation Guide to assist you in booking a hotel accurately for your reimbursement form.

Conference or Workshop Attendance Request Form

Step one

Complete the Conference or Workshop Attendance Form.

Step three

Your supervisor will either approve or deny the conference attendance request based on available funding.

Step five

You will receive a notification regarding the final approval. If approved, make sure take care of your registration and also book your accommodations and travel needs.

Step two

Submit the completed Conference or Workshop Attendance Form to your supervisor.

Step four

If your supervisor approves, the form is forwarded on to the Agency Administrator for approval.

Step six

Enjoy your conference and submit any necessary expenses using the expense reimbursement procedure and form.
