CESA 2 Cell Phone Program or Reimbursement Request

Interested in participating in the CESA 2 cell phone program or need to make a change to your cell phone service or equipment? Looking to receive reimbursement for using your personal cell phone for agency business? Eligible employees may be able to be reimbursed for use of their personal cell phone instead of being assigned an agency phone. Use the forms below to start the process.

List the cell phone number you will use for business purposes

I agree to submit the first page of my monthly cell phone statements (which indicates the monthly billed amount) to the appropriate Accounting Associate. If I do not submit monthly cell phone statements, I understand I will not be reimbursed.

I have read the CESA 2 Technology Acceptable Use Policy and the Cellular Phone Program Policy and I agree to follow the rules contained in each. I understand that if I fail to comply with these policies, CESA 2 administration will withdraw the privilege of participating in the cellular phone reimbursement program. In addition, I may face other disciplinary action and/or action to recoup money to the agency. Employee signature also authorizes deductions as necessary in accordance with the policy.
